Local phone service

Review the list of things to consider before placing your order. Include your answers to the questions in your order.

To add, change or move telephone service, send an Intradepartmental Request (IR) or Service Request to OIT via:

  • fax (684-3383) or
  • campus mail (Box 90217)

  • make and model of the phone(s)
  • the telephone number(s)
  • the user(s) name(s)

To request or cancel a long distance calling card:

To request or cancel a long distance authorization code:
